Has anybody else experienced this, and what may be the reason?

I'm sorry to hear that your dental implants failed.
Unfortunately, any time you introduce something foreign to the body, you run the risk that the body will not adapt to it. I was told by my periodontist that dental implants have a 5% failure rate.
My first dental implant failed due to infection. Infection and insufficient bone can contribute to failure. However, you mention that your husband's bones were deemed suitable for implantation. Mine were as well.
Sometimes the implant just doesn't take. My periodontist said this is perplexing, because there is really only one place to put the implant, so it's not like you can get it wrong. I would advise getting your husband checked out for dental infection. It is also possible that the implants just didn't take. Your best bet is to get to a periodontist as soon as possible.
